New Student Enrollment Information

What to Bring for Registering my Student
 
 

Items Needed for School Registration

Parent/Legal Guardian must accompany the student to school and bring the following:

1. State Issued certified copy of Birth Certificate of student
 
2. Student's Social Security Card
 
3. Immunization Record of Student (See list of updated immunization requirements below).
 
4. 3 forms of Proof of Residency (Living in the City of Bristol, VA)
 
5. Unofficial Transcript from former school. Parent should give the former school at least a day's notice to prepare an unofficial transcript. Schools are required by law to give parent's a copy of their children's record. 
 
6. Custody/Guardianship papers. Parents granted custody through divorce must provide a copy of custody papers. The same regulations apply to legal guardians who have been appointed custody by the court. 
 
7. Entrance into school form must be filled out by a physician. Students will be conditionally enrolled if they can show an appointment card which indicates an appointment has been scheduled for a physical examination at a later date with the Bristol VA Health Department. If the physical examination is not kept on that date, students will not be permitted to return to school after that date (Form can be found in enrollment packet).
