School Tdap Immunization Clinic for 6^th grade February 28,2020

Virginia state law requires that all students entering 7^th grade must have received a booster dose of pertussis-containing vaccine, or the Tdap (tetanus, diphtheria, and pertussis vaccine).  The Virginia Department of Health will be @ Virginia Middle School on February 28,, 2020 to administer the TDAP vaccine to those students whose parent/guardian would like for their child to receive the vaccination @ School.   In order to receive the vaccine @ school, please complete the vaccination consent form and have your child return the form to the nurse @ Virginia  Middle School.  

If your child has already received this vaccine, please bring a copy of your vaccination record to the school nurse @ Virginia Middle School.  Rising 6^th graders will not be allow to start 7^th grade in August, without having received this vaccination.
