Virginia Public Schools will conduct a
centralized student registration for the
2009-2010 school year. All new,
incoming students will need to register
at the school board office regardless of
the school in which they will attend in
the fall. |

Student Registration Form
Declaration of Residence Form
At least three forms of identification
must be supplied with this form.
Type of identification includes:

Current original signed lease agreement or
mortgage documentation |

original water, gas. or electric department
invoice |
State or federal tax return filed within the

original tax assessment statement |
Check stub from current employment (current
address and resent month) |
| Monthly bank
statement (past 2 months) |
Homeowner's Insurance Policy |
| Medical
Bills (must include name and current

Social Security documentation that includes

School Entrance Health Form

Information Form must be completed by
